Health Assessment
Our intelligent Health Assessment design does more than just identify risks, it drives personalization of the entire online experience. The Health Assessment, developed by the University of Michigan Health Management Research Center, builds a tailored health profile for consumers based on their current health, health history, lifestyle habits and perception of well-being. Users are guided through a series of 52 questions, allowing for measurement of more than 30 different mortality and morbidity indicators.
Health Intervention
The Optum lifestyle intervention program combines technology with personal health coaching to achieve measurable health improvements for those at risk. This proactive approach will prevent more individuals from entering the high-risk, high-cost, chronically ill category. Qualified participants are matched with a health behavior specialist for personal coaching by telephone. This coaching is tailored to each participant's readiness to change. Educational mailings and online education kits are also available.
Along with helping individuals identify their risks, the program also gives management the information necessary to show the risk levels for the entire population and assign a dollar figure to assess health care costs based on unhealthy lifestyle factors. This information helps you target programs so your health management resources are spent in the areas you need them the most.
Integration with NurseLineSM and Connect24
Our superior clinical and operational expertise allows us to integrate health assessments with our NurseLineSM and Connect24® services because it is necessary to evaluate an individual's spectrum of health needs now and in the future. High-risk individuals may get a reminder to complete a health assessment or a referral to an appropriate lifestyle intervention program. This way, we get individuals to take one extra step in identifying their health needs or issues and provide them with the resources they need to stay healthy, get healthy or live with illness.
